Responsibilities
- Demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.
- Acts as a health coach, helping patients set short and long-term health goals, and frequently follows up on those goals.
- Simplifies and prioritizes appropriately so that behavior change is more actionable.
- Engages with the hospitalist whenever one of their patients is in the hospital.
- Assesses, diagnoses, treats, manages, educates, promotes health and coordinates care for patients with acute and complex chronic health needs.
- Leads their care team consisting of a care promoter, care facilitator, and care coordinator for patients able to come to the office. Engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources for patients unable to come to the office.
- Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP).
- Takes an active role as needed in recruiting patients for the center and additional providers for the company.
- Works closely with the applicable managers and medical directors to ensure compliance with guidelines, participates in risk and quality management programs, clinical meetings, and other meetings as required.
- Adheres to departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
Requirements
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working.
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required.
- Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required.
- Has a current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment.
Qualifications
- Fluent in English, verbal and written.
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes.
- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company.

Pay
The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
