About the role

As part of Hartford HealthCare at Home, the largest provider of homecare services in Connecticut, this role provides skilled nursing care to clients in home and community-based settings. The role collaborates with all team members to achieve exceptional outcomes.

Responsibilities

• Maintains a client record utilizing the Agency’s standard for documentation.
• Makes follow-up visits to provide skilled care to assess disease progress and evaluate response to treatment.
• Provides support to patients and families using teaching principles to promote and restore health and prevent disease and disability.
• Coordinates care with providers.
• Calls on and collaborates with all team members involved in the patient’s care.
• Consults with physicians regarding client needs and plan of care.
• Communicates and conferences with Nurse Care Coordinators and other involved in the patient’s care.
• Directs LPNs in carrying out the established plan of care.
• Directs Home Care Aides and supervises the plan of care.
• Refers clients and families to other community resources as needed.
• Attends and participates in in-service education programs.

Requirements

• Graduate of an Associate’s Degree or diploma program in nursing.
• One year nursing experience or one year nursing experience in a specialty.
• Registered nurse with a license to practice in the state of CT.

Qualifications

• Preferred: Graduate of NLN approved BSN program.
• Prior medical/surgery or home care experience preferred.
