Primary Care Nurse RN or LPN

Role Overview

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient‑centered care in a systematic way. The nurse partners with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties are performed in alignment with Mercy’s mission, values, and Service Standards.

Responsibilities

• Work closely with Primary Care Providers to manage triage calls and messages for the clinic.
• Provide patient‑centered care and support quality outcomes for an attributed panel.
• Participate in monthly on‑site clinic meetings.
• Complete a minimum of three months of on‑site training before transitioning to a hybrid schedule.
• Maintain accurate documentation using the Epic EMR system.

Qualifications

LPN

• Diploma in Practical Nursing.
• Active LPN license.
• 2–3 years of experience preferred.

RN

• Associate of Applied Science in Registered Nursing, graduation from a registered nursing program, a BSN, or 4 years of comparable experience in lieu of a BSN.
• Active RN license.
• 2–3 years of experience preferred.

Highly Preferred Skills

• Proficiency with Epic EMR system.
• Timely and thorough patient triage skills.
• Strong computer competency (the position is primarily computer‑based).
• Excellent time management and prioritization.
• Adaptability in a new and changing environment.

Schedule

• Monday – Friday, 08:00‑16:30.
• No nights, weekends, or holidays.
• After training, a 50/50 hybrid model (on‑site five days per pay period) with monthly on‑site clinic meetings.
