We are hiring a Primary Care Nurse Practitioner / Physician Assistant for a full-time outpatient opportunity in Hartford, CT. What You’ll Do
Provide comprehensive primary care visits, assessments, and ongoing management for adult patients
- Diagnose and treat acute and chronic medical conditions
- Order and interpret diagnostic tests and labs
- Prescribe medications and manage treatment plans as appropriate
- Educate patients on health maintenance, prevention, and chronic disease management
- Collaborate with physicians, nursing staff, and the broader care team to ensure continuity of care
- Maintain accurate, timely documentation within the EMR
What We’re Looking For
- Active or eligible Connecticut licensure as a Nurse Practitioner or Physician Assistant
- Prior experience in Primary Care, Family Medicine, Internal Medicine, or a related outpatient setting preferred
- Strong clinical judgment, communication, and patient education skills
- Comfortable managing a broad adult primary care panel
- Mission-driven, team-oriented, and committed to quality community-based care
