We are hiring a Primary Care Nurse Practitioner / Physician Assistant for a full-time outpatient opportunity in Hartford, CT. What You’ll Do

Provide comprehensive primary care visits, assessments, and ongoing management for adult patients

  • Diagnose and treat acute and chronic medical conditions
  • Order and interpret diagnostic tests and labs
  • Prescribe medications and manage treatment plans as appropriate
  • Educate patients on health maintenance, prevention, and chronic disease management
  • Collaborate with physicians, nursing staff, and the broader care team to ensure continuity of care
  • Maintain accurate, timely documentation within the EMR

What We’re Looking For

  • Active or eligible Connecticut licensure as a Nurse Practitioner or Physician Assistant
  • Prior experience in Primary Care, Family Medicine, Internal Medicine, or a related outpatient setting preferred
  • Strong clinical judgment, communication, and patient education skills
  • Comfortable managing a broad adult primary care panel
  • Mission-driven, team-oriented, and committed to quality community-based care
