As a Primary Care Provider, you'll serve as a lifeline for homebound seniors, delivering relationship-based care, managing chronic and acute conditions, addressing social determinants of health, and driving improvements in health outcomes.

What You’ll Do

• Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with support from clinical operations, triage team, and MD leadership.
• Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
• Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
• Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
• Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
• Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.

Requirements

• A NP or PA with a master’s degree and active Texas licensure
• Passionate about serving seniors and medically complex populations
• Skilled at building trust, communicating clearly, and adapting to home-based environments
• Motivated by mission, relationship-driven care, and meaningful outcomes

Benefits

• $100,000 – $125,000 annually, depending on experience, with opportunity for performance-based bonuses and full benefits
