About the role

Bloom Healthcare is an award-winning, employee-owned medical practice dedicated to providing high-touch, high-quality care to medically complex patients in their homes and communities. We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Highlands Ranch, CO.

Responsibilities

  • Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities
  • Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership
  • Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers
  • Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments
  • Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers
  • Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes
  • Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients

Requirements

  • A NP or PA with a master’s degree and active Colorado licensure
  • Passionate about serving seniors and medically complex populations
  • Skilled at building trust, communicating clearly, and adapting to home-based environments
  • Motivated by mission, relationship-driven care, and meaningful outcomes

Qualifications

  • Experience in primary care, preferably in home and community settings
  • Strong clinical skills and knowledge of medical practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively

Skills

  • Effective problem-solving and decision-making skills
  • Strong organizational and time management skills
  • Proficiency in electronic health records (EHR) systems

Pay

$100,000 – $125,000 annually, depending on experience, with opportunity for performance-based bonuses and full benefits.

Schedule

Monday - Friday
8-12 patients per day (no unrealistic volume expectations)
Light call rotation: only 2–3 times per year
Weekends off and flexible daily workflows
