Primary Care Nurse Practitioner or Physician Assistant

Responsibilities
- Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities
- Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership
- Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers
- Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments
- Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers
- Utilize Bloom’s integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes
- Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients
Requirements
- A NP or PA with a master’s degree and active Colorado licensure
- Passionate about serving seniors and medically complex populations
- Skilled at building trust, communicating clearly, and adapting to home-based environments
- Motivated by mission, relationship-driven care, and meaningful outcomes

Pay
$100,000 – $125,000 annually, depending on experience, with opportunity for performance-based bonuses and full benefits.
Schedule
Monday - Friday
8-12 patients per day (no unrealistic volume expectations)
Light call rotation: only 2–3 times per year
Weekends off and flexible daily workflows
