Primary Care Nurse Practitioner – Complex Chronic Care

About the role

A value-based company in the Virginia Beach/Norfolk area is seeking a Primary Care Nurse Practitioner to join their team.

Responsibilities

• See patients in dialysis clinics (approximately 5‑7 per day)
• Manage patients with complex chronic illnesses
• Serve as primary care provider for patients with kidney failure
• Utilize telehealth when clinics are geographically spread out, though preference is for in‑person, face‑to‑face visits

Schedule

Team generally arrives around 7:00 AM, leaves around 3:00‑4:00 PM, and completes charting from home afterward.

Qualifications

• At least two years of experience as a Primary Care Nurse Practitioner
• Experience managing complex chronic conditions
• Nephrology experience is preferred but not required

Pay

Base salary $140,000‑$150,000 plus bonus potential plus benefits.
