Primary Care - Board Certified Nurse Practitioner/Physician Assistant

About the role

The Physician Assistant / Nurse Practitioner provides primary health care services to patients of Community Health Center under the general supervision of the Chief Medical Officer. The primary purpose of CHC is to provide the highest quality of total care possible to the patient population it serves. This quality depends on the staff's desire and ability to work together, individually, and as a team. The employee is expected to be professional, punctual, maintain regular attendance, cooperative, organized, and enthusiastic at all times.

Responsibilities

  • Provide comprehensive primary care services including but not limited to history taking, physical examination, diagnosis, treatment, preventive care, and patient education.
  • Collaborate with other healthcare professionals to ensure continuity of care and meet patient needs.
  • Document patient encounters in an accurate and timely manner using electronic health records.
  • Participate in quality improvement initiatives and ongoing professional development activities.
  • Adhere to established policies and procedures related to patient care, safety, and infection control.

Requirements

  • Current licensure as a Physician Assistant or Nurse Practitioner in the state of [State]
  • Completion of accredited physician assistant or nurse practitioner program
  • At least 1 year of relevant clinical experience
  • Excellent interpersonal and communication skills
  • Ability to work independently and as part of a team

Qualifications

  • Master’s degree in Physician Assistant or Nurse Practitioner from an accredited program
  • Current certification in Advanced Cardiac Life Support (ACLS)
  • Passing score on the National Certification Exam for Physician Assistants or Nurse Practitioners
