Primary Care Assistant LPN - FT, Days
About the role
Under the direction of the primary care operations team, the Primary Care Patient Care LPN works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Patient Care LPN will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.
Responsibilities
- Collaborate with Mercy Primary Care Providers to deliver patient-centered care
- Partner with patients, providers, and clinical care team members to drive quality outcomes
- Perform duties consistent with Mercy’s mission, values, and service standards
Requirements
- Graduate of an accredited practical nursing program
- Licensed as an LPN (Licensed Practical Nurse) in the state of practice
- 6 months clinical healthcare experience, preferably in a primary care office (preferred)

Physical Requirements
- Push, pull, and/or lift 50 pounds on a regular basis
- Prolonged standing and walking each shift
- Ability to grip, reach, bend, kneel, twist, and squat to perform duties
