About the role

This position is a HYBRID role supporting primary care in Spencer, IN and virtual support for other offices within the region. The position will work part-time in person at the Spencer, IN office and part-time virtually. Candidate may select from working 5, 8-hour days or 4, 10-hour days.

Provides comprehensive, value-based medication-related care to achieve definite outcomes that improve a patient's quality of life. Acts in collaboration with other healthcare professionals in the shared responsibility for patient medication therapy outcomes.

Responsibilities

• Ensuring safety and effectiveness of the medication use process
• Medication therapy monitoring
• Regulatory compliance
• Protecting the confidentiality of patient information
• Providing medication information support for medical and nursing staff, ensuring continuity of care
• Identifying potential challenges and intervening, if necessary, to medication-related issues

Requirements

• Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006
• Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required
• 0-3 years of relevant experience
• Current Indiana Pharmacist license or eligibility for Indiana licensure as a Pharmacist
• Board certification or other specialized training may be required
• Basic Life Support certification through the American Heart Association (AHA) may be required

Schedule

Hybrid schedule: part-time in person at the Spencer, IN office and part-time virtually. Candidate may select from working 5, 8-hour days or 4, 10-hour days.
