Primary Care Advanced Practice Provider

About the role

We are seeking Primary Care Advanced Practice Providers for our Rhode Island locations. Schedule: Monday-Friday, 8am-5pm (no nights or weekends). 12-16 patients per day once fully ramped up (could start lower if new site).

Responsibilities

• Treatment of a primarily Medicare (over age 65) patient population, with some sites having up to 25% under age 65.
• Collaborative care with a MD, APP, Scribe, MA, and shared RN/Social Work support.
• Rapid consultation with specialists through a telemedicine partnership.
• Use of EPIC EMR with support from scribes and advanced algorithms to optimize patient care and reduce hospitalizations.
• Rotated call schedule with 1 week every 3 months, with a call center available to triage calls remotely.

Requirements

• Board Certification Required
• R.I. State License Required
• Training as a Family NP or Adult NPPrevious primary care or skilled nursing facility/nursing home experience preferred, but not required.

Qualifications

• Experience in a value-based care model aimed at reducing hospitalizations for Medicare patients.

Pay

• Hourly rates and/or salaries listed may or may not reflect total compensation packages including bonus and fringe benefits, etc.

Schedule
