Primary Care Advanced Practice Provider

Key Responsibilities

• Examine, obtain medical history, and treat acute and chronic illnesses.
• Collaborate with management, other physicians, and staff to ensure comprehensive patient care.
• Adhere to Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives and guidelines.
• Provide on-call coverage within the primary care group.

Requirements

• Master's degree as a Nurse Practitioner or Physician Assistant.
• Connecticut State license, DEA, and CSR (active or applied preferred but not required).
• Minimum of 1 year of experience.

Note

The call schedule is 1:8, requiring calls from home and a 4-hour block on Saturday mornings every 8 weeks. Flexibility is needed for those interested in a 4-day work week.
