HealthPro Heritage has a full-time opportunity for a Licensed Physical Therapist Assistant (LPTA) at Presidential Oaks in Concord, NH, a long-term care setting.

Responsibilities
- Assist in implementing treatment plans designed by Physical Therapists.
- Provide direct patient care, including therapeutic exercises, manual therapy, and modality applications.
- Monitor and document patient progress, reporting observations to the Physical Therapist.
- Educate patients and families on treatment protocols, exercises, and self-care techniques.
- Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
- Collaborate with Physical Therapists, healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
Requirements
- Associate’s degree in Physical Therapist Assisting from an accredited program.
- Valid state licensure as a Physical Therapist Assistant (or license eligible).
- Strong assessment, diagnostic, and therapeutic abilities.
- Excellent communication and interpersonal skills.
- Proficiency in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems.
- Ability to work collaboratively in a multidisciplinary team.
Pay
$33–$40 per hour, based on years of experience.
Schedule
Full-time, stable schedule.
