Job Summary
To act as patient advocate, providing continuity of care designed to meet individual patient and facility needs through collaboration with other members of the healthcare team and supporting the philosophy, objectives and goals of the facility.
About the Role
Assesses the needs of individual patients, documenting and reviewing their history including adult, pediatric, adolescent, and geriatric patients.
Essential Functions
- Formulates a goal-directed plan of care based on determined nursing diagnoses and patient outcomes
- Evaluates and assesses patient's history and physical, medical clearance, and patient labs with the provider's office and anesthesia staff
- Implements care in a knowledgeable, skilled, consistent manner to achieve desired patient outcomes through nursing processes
- Performs preoperative and postoperative phone calls to gather patient medical information and health history
- Documents patient care in a timely, accurate, and concise manner
- Works as part of a team in delivering patient care
- Maintains BLS certification and attends all mandatory in-services and staff meetings
- Accepts and implements new ideas and approaches, treats all patients equally, and follows the center's Patient Rights policy
- Willingly assists with other duties as asked
Physical Demands
- Frequently sits, converses, and listens
- Uses hands to touch, handle, or feel objects, tools, or controls
- Adjusts focus for close vision
- Lifts and/or carries over 20 pounds on a regular basis
- Pushes/pulls over 25 pounds on a regular basis
- Stands and/or walks for at least 10 hours per day
Work Environment
- The work environment is characterized by frequent interaction with others, frequent walking or running, frequent sitting, frequent standing, frequent lifting/carrying up to 20 pounds, and frequent pushing/pulling up to 25 pounds
