The Practice Care Coordinator

Responsible for supporting primary and specialty care objectives in outreach to patients, coordination of care with managers, and proactively creating relationships with physicians/advanced practice practitioners and patients to ensure optimal health and wellness for the patient population.

  • Supports clinical care management and Primary Care activities, including care transitions activities to improve care outcomes for eligible at-risk patients.

Required Minimum Knowledge, Skills, And Abilities (KSAs)

  • Education: Graduate of an accredited Medical Assistant program OR completion of EMT-Basic, EMT-Advanced or Paramedic training required.
  • Licenses/Certifications:
    • New Hampshire Residents: National Registry AEMT or Paramedic License and/or NH AMET or Paramedic License.
    • Maine Residents: National Registry AEMT or Paramedic License.
  • Certified Medical Assistants: Certification and a New Hampshire Tech License required.
  • BLS certification required or must obtain within 30 days of start date.

Experience

  • One year of experience in a medical facility as a Medical Assistant and/or documented evidence of an externship completed in a medical facility required.
