Population Health Pharmacist

About the role

The Population Health Pharmacist is responsible for optimizing medication therapy outcomes across targeted patient populations through evidence-based clinical interventions, medication management, and collaborative care strategies. This role supports organizational population health initiatives by improving quality metrics, reducing healthcare costs, enhancing medication adherence, and promoting safe and effective medication use.

Requirements

• Doctorate of Pharmacy
• Licensed or eligible for licensure in the state of Pennsylvania
• Ability to expertly communicate verbally and in writing including the use of electronic mail
• Excellent human relations skills
• Able to interact in a professional manner
• Able to delegate and motivate others within the department to act on performance improvement opportunities
• Able to adapt to change
• Knowledge of the law as it applies to outpatient practices

Preferred

• Management experience
• Experience and knowledge of population health strategy
• Ambulatory pharmacy experience
• Competency with Microsoft Office suite
• Understanding of value based reimbursement programs
• Advanced degree in Pharmacy or Business
