Population Health Nurse Practitioner NP | Telehealth, RPM & CCM

About the role

Build meaningful, long-term relationships with Medicare patients
Practice proactive medicine instead of episodic urgent care
Practice at the top of your license through advanced clinical decision-making
Work alongside dedicated care coordinators who manage routine outreach and monitoring
Collaborate closely with physicians, home health, hospice, and palliative care teams
Help shape an innovative Population Health program focused on improving outcomes and reducing hospitalizations

Responsibilities

• Scheduled telehealth follow-up visits with established Medicare patients
• Quarterly comprehensive chronic disease reviews
• Clinical evaluation of patients identified through RPM and CCM programs
• Medication optimization and chronic disease management
• Treatment plan adjustments
• Collaboration with physicians and interdisciplinary care teams

Requirements

• Current Nurse Practitioner license (Arizona preferred)
• National Board Certification (FNP, AGNP, ANP, or equivalent)
• Excellent communication and interpersonal skills
• Strong clinical judgment and critical thinking
• Comfortable using telehealth platforms and electronic medical records

Qualifications

• Preferred: Primary Care, Family Medicine, Internal Medicine, Geriatrics, Population Health, Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Telehealth, Value-Based Care, Medicare population experience, Multiple state licenses
