Population Health Care Manager - Rising Risk

About the role

The Population Health Care Manager - Rising Risk is responsible for managing patient populations with chronic conditions to improve health outcomes, reduce unnecessary utilization, and ensure coordinated care across settings.

Responsibilities

• Develop and implement individualized care plans
• Perform disease management and support transitional care
• Connect patients to community resources
• Facilitate communication among care team members
• Conduct assessments and monitor interventions
• Promote patient activation and self-management
• Engage with patients, families, and providers via in-person, community, and telehealth settings, including home visits when needed
• Participate in interdisciplinary collaboration and quality improvement activities
• Work independently in a fast-paced environment

Requirements

• Bachelor’s or Master’s degree in nursing, counseling, social work, or related field
• Relevant clinical experience
• Licensure by NC Boards and case management certification required
• Exceptional communication, problem-solving, and organizational skills
• Proficiency in documentation and care coordination
• Emphasis on cultural sensitivity and patient-centered care
