Psychiatric Nurse Practitioner

MindCare Solutions is seeking a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our dynamic team on a part-time basis (potential to grow to full-time). As a leading prover of mental health services, we are committed to providing exceptional care and support to our geriatric patients and residents who are facing mental health challenges in the LTC community. Essential Duties And Responsibilities

• Conduct Comprehensive Psychiatric Evaluations: Perform thorough psychiatric assessments to establish a baseline, manage psychotropic medications, and deliver personalized, need-based care.
• Ongoing Medication Management: Develop and adjust treatment plans, including medication management, based on patient progress and input from the interdisciplinary team.
• Collaborative Care Planning: Partner with case managers and interdisciplinary teams to create and refine care plans, ensuring a seamless, integrated approach to behavioral health management.
• Engagement in Quality Programs: Contribute to quality initiatives such as performance improvement programs, medication reviews, and outcome optimization strategies.
• Travel: Travel up to 50% of the time to meet with healthcare partners, attend industry events, and provide consistent support across clinical settings.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health
• Current and unrestricted PMHNP license in the state of OH
• Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
• Active and unrestricted DEA (Drug Enforcement Administration)
