SUMMARY

The Resident Care Partner is responsible for providing personal assistance and routine daily care and services related to the residents in accordance with the Resident Care Profile, and as may be directed by the Executive Director and Wellness Director in order to enhance the residents’ independence and quality of life. Reports to: Wellness Director. Hours: 7pm-7am. Location: The Vincent Senior Living.

ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES

  • Assist residents in their Activities of Daily Living (ADL) tasks as needed and directed. This includes bathing, grooming, personal and dental hygiene, transportation to meals and activities, bowel and bladder functions (e.g., take to bathroom, portable commode, change incontinence products, etc.), shaving, dressing, and hair care.
  • Respond to resident’s needs promptly while maintaining each resident’s self-respect, personal dignity, personal safety and confidentiality.
  • Absorb residents into and out of bed and wheelchair as needed, following safe lifting/transferring techniques.
  • Absorb residents at mealtime, providing assistance with serving meals and clearing tables after each meal, as directed. Deliver room trays, as directed.
  • Encourage residents to participate in daily activities such as bed making, laundry, and setting tables.
  • Encourage and assist residents in keeping rooms neat and maintaining a safe environment.
  • Communicate any observed or suspected resident change of condition to a supervisor immediately.
  • Respond timely to resident pull cords/alerts. Carry walkie-talkie/mobile device on person and respond as required.
  • Assist Activities Department staff in carrying out planned activities. Assist residents in preparing for activity and social programs.
  • Absorb that resident care sign-off sheets are completed by the end of each shift.
  • Absorb new residents with arranging personal items and help to familiarize them to new surroundings, by introducing them to other residents.
  • Use Standard Precautions and follow established policies concerning exposure to blood/body fluids.
  • Assure that resident care sign-off sheets are completed by the end of each shift.
  • Display tact and friendliness when dealing with residents, families and visitors.
  • Attend scheduled in-service classes and mandatory staff meetings.
  • Maintain all required certifications/licenses as required by state law.
  • Provide support to community’s marketing efforts through positive/friendly interaction during all property tours and telephone contact with the public, and as directed by a supervisor.
  • Maintain a safe and secure environment for all staff, residents and guests, following established safety standards.
  • Encourage teamwork through cooperative interactions with co-workers and other departments.
  • Support a positive and professional image through actions and dress.
  • Must follow the dress code.
  • Performs other duties consistent with the position as assigned by the Wellness Director.
  • May float to Memory Care Center as business needs arise.
  • Provide services consistent with care plans/service plans.
  • Report unusual occurrences and incidents per policy.
  • Anticipate needs of residents and report variances in care appropriately.
  • Perform work as scheduled, understanding work schedules may change due to business needs.
  • Perform other duties consistent with the position as assigned by the Wellness Director, Memory Care Director (if in Memory Care), or Executive Director.

MINIMUM QUALIFICATIONS

  • Education: Must be 18 years of age or older. High school diploma or equivalent. May require additional certification such as Certified Nursing Assistant or Certified Home Health Aide as required by state or board regulations.
  • Experience: One-year experience in a health care environment or equivalent preferred.
  • Mental Skills: Ability to do simple mathematical calculations. Reasoning Skills: Ability to work patiently with cognitively impaired individuals. Oral/Written Communication Skills: Ability to read, write, and speak English Ability to follow oral and written directions.
  • Equipment Used: Standard household equipment (dishwasher, iron, vacuum cleaner, washer, dryer).
  • Physical Effort: Must be mobile. Ability to assist cognitively impaired people with activities of daily living. Must be physically and mentally capable of performing assigned tasks.
  • Personal Characteristics: Possesses good character and displays high degree of personal integrity. Ability to work with others in providing care. Ability to show sincere compassion towards cognitively impaired older adults. Demonstrates genuine concern for the physical and emotional needs of older people and their families.
  • Working Conditions: Air-conditioned, well-lighted health care environment. In Memory Care Center, ability to work in secured area with heavy interaction with a cognitively impaired population.
