Licensure & Certifications
- Board Certified or Board Eligible in Family Medicine
- Licensed in Indiana or eligible for medical licensure in Indiana

- Excellent communication skills and ability to collaborate with the care team
- Ability to develop and maintain relationships with referring physicians
