About the role

We are seeking a highly skilled and compassionate PACE Physician to join our team. The PACE Physician will be responsible for providing primary care services to our elderly patients in a Program of All-Inclusive Care for the Elderly (PACE) setting. The ideal candidate will have a passion for working with the elderly population and a commitment to providing high-quality care.

Key Responsibilities

  • Provide primary care services to elderly patients in a PACE setting
  • Conduct comprehensive assessments of patients' medical, social, and functional status
  • Develop and implement individualized care plans for each patient
  • Cookordinate care with interdisciplinary team members, including nurses, social workers, and therapists
  • Monitor patients' health status and adjust care plans as needed
  • Provide education and counseling to patients and their families
  • Participate in quality improvement activities and initiatives

Qualifications

  • All applicants must have a medical degree from an accredited institution
  • All applicants must be board certified in Internal Medicine or Family Medicine
  • All applicants must have an active medical license in Missouri
  • Experience working with elderly patients is preferred
  • All applicants must have excellent communication and interpersonal skills
  • All applicants must be able to work effectively in a team-based care model
  • All applicants must have a commitment to providing high-quality, patient-centered care
