Key Responsibilities

• Initiates an appropriate assessment of person health status, ranging from focused to comprehensive.
• Identifying immediate needs which can be supported or the need for a referral for long term management.
• Performing and documenting a health history as evidenced by chart review and ability for others to understand patient needs.
• Consulting with colleagues, specialists, and other health professionals as appropriate.
• Ordering pertinent diagnostic tests and procedures to support recommended therapy.
• Supporting obtaining or performing specimen collection for commonly performed blood, urine, and stool analyses and cultures.
• Interpreting health information and establishing diagnoses consistent with the accumulated data and formulating treatment plan, ensuring timely provision of necessary health care services.
• Providing education and ensuring understanding of members regarding the nature of the identified acute needs and further opportunities for evaluation and treatment.
• Participating in the supervision of mid-level providers, including Nurse Practitioners/Physician Assistants.
• Educating patients regarding health promotion and disease prevention.
