This role serves primarily as a steward of population health through Eventus WholeHealth’s Outcomes-Centered Healthcare (OCH) model. The Attending Physician provides leadership in client facilities and collaborates closely with Advanced Practice Clinicians (APCs) to achieve excellence in patient care and professionalism in client relations. The position ensures high-quality, accountable, team-based care across assigned facilities. Physicians may also serve in a designated Medical Directorship capacity, providing clinical leadership and oversight within one or more facilities.
Responsibilities
- Lead and oversee population health initiatives within the Outcomes-Centered Healthcare (OCH) model.
- Collaborate with Advanced Practice Clinicians (APCs) and interdisciplinary teams to deliver high-quality, team-based care.
- Provide clinical leadership and oversight in assigned facilities, including potential Medical Directorship responsibilities.
- Ensure excellence in patient care and professional client relations.
- Utilize EMR systems (AthenaHealth preferred) and care coordination workflows.
Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
- CMD certification a plus; required within 2 years if serving in a Medical Directorship capacity.
- Active and unrestricted medical license in applicable state(s); DEA registration as required.
- Minimum 2–3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred.
- Experience collaborating with interdisciplinary teams and facility leadership.
