About the role

The Center Medical Director provides medical care for InnovAge PACE enrolled participants across various settings, including the center's clinic, participants' homes, nursing homes, assisted living facilities, and personal care boarding homes. Close coordination with hospital-based providers is essential.

Responsibilities

  • Provide medical care to enrolled participants, including evaluations, diagnoses, and management of medical conditions.
  • Document medical care in the participant’s medical record, in compliance with documentation policies.
  • Participate within the interdisciplinary team in the formulation of Plans of Care for participants.
  • Assist with the preparation of the primary care budget and explain monthly budget variances.
  • Respond to center grievances with respect to primary care complaints.
  • Champion at least one primary care strategic initiative.
  • Coordinate regular primary care team meetings at the center and lead local meetings when asked.
  • Recruit, hire, and terminate primary care staff under the supervision and in consultation with the Area Medical Director.
  • Supervise and manage direct reports, providing motivation and direction to employees.
  • Schedule leadership/management, department, and staff 1:1 meetings to ensure coordination and collaboration in meeting organization and individual goals.

Requirements

  • Doctor of Medicine or Osteopathy degree.
  • Board Certified in a primary care specialty (Board Eligible but must pass boards within 12 months of employment).
  • Current active Medical License for state in which practicing.
  • DEA License with full prescriptive authority.
  • Adequate malpractice coverage.
  • Not on the Preclusion List.
  • A minimum of one year working with the frail or elderly.
  • PREFERRED: Postgraduate training in a primary care specialty, including but not limited to internal or family medicine.
  • Completion of post-residency training in geriatric medicine.

Qualifications

  • Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential.
  • Must be able to perform physical tasks associated with the job.

  • Knowledge of PACE program and its requirements.
