We are seeking a motivated Physician to deliver on-site medical care in skilled nursing and long-term care facilities across the Portland, OR market. Full-time and part-time positions are available, offering a predictable schedule with no nights or on-call coverage. You’ll receive strong clinical and specialty support, backed by experienced Nurse Practitioners and Physician Assistants who manage follow-up care, allowing you to focus on complex clinical decision-making and patient outcomes without administrative overload.

Responsibilities

• Round on patients in skilled nursing and long-term care settings
• Conduct history and physical examinations as well as monthly regulatory visits
• Collaborate with NPs, PAs, nursing staff, and facility leadership
• Coordinate care across a multidisciplinary team
• Utilize AI-integrated EHR documentation for efficient charting

Requirements

• Current, unrestricted MD or DO license in the applicable state
• Board certification in Internal Medicine, Family Medicine, and/or Geriatrics preferred
• SNF experience is a plus, but not required
