About the role
The primary care physician (PCP) provides comprehensive medical care to elderly participants within a multidisciplinary team setting.
Responsibilities
- Conduct assessments and develop and implement personalized care plans
- Manage participant care in nursing homes
- Coordinate specialist referrals and oversee hospitalizations
- Actively participate in team meetings and communicate participant changes
- Review clinical data and suggest improvements to reduce unnecessary hospitalizations
- Handle documentation and diagnostic coding
- Contribute to policy development and staff training
- Perform quality assurance activities
Requirements
- MD or DO license
- Board certification in Internal Medicine or Family Practice
- Experience with elderly populations
- Current certifications: CDS, DEA, and BLS
