Responsibilities
- Serves adults for metabolic health and obesity medicine.
- Might share a panel with MD/DO and other APCs in local in-clinic practices.
- May see pediatric patients ages 12+ for metabolic health only, based on training, licensure, and competency.
- Delivers hybrid care, combining in-person and remote patient care.
- Uses audio and visual technology for remote patient examinations, evaluations, assessments, diagnoses, and treatment plans.
- Embodies knownwell's mission and values of patient inclusivity.
- Collaborates with the broader care team to ensure patient support.
- Documents accurately for billing purposes, including reviewing remote patient monitoring data.
- Communicates with patients via the portal for questions and concerns.
- Ensures patient panels are open to individuals regardless of their health status and comorbidities.
- Encourages patients and tailors care to their unique health journeys.
- Provides an environment free from judgment and encourages patients to reach their goals.
- Escalates concerns to the dedicated site leadership team.
- Provides clinical guidance to advanced practice clinicians, medical assistants, and nursing teams.
Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited medical school.
- An active, unrestricted license to practice in Illinois, or willingness to obtain.
- Board certification in Family Practice or Internal Medicine.
- Preferred certification with American Board of Obesity Medicine.
- Valid DEA with no restrictions and prescribing authority (per state practice regulation).
- BLS certification current.
- Two (2) years of in-person clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.
- If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue ABOM certification.
- Willingness to supervise Advanced Practice Clinicians as deemed necessary and appropriate by the practice.
- Abides by state-specific rules and regulations as a collaborative physician.
- Experience networking and promoting self and the practice through individual and group outreach campaigns.
- Excellent communication and interpersonal skills.
- Dedicated to staying current with the latest developments in primary care and obesity medicine.
- Believes in inclusive, compassionate care and shared decision-making with patients.
- Honors patients' goals for their health and partners with them to help them achieve those goals.
- High emotional intelligence and ability to make patients feel at ease – without judgment and without shame.
- Willing and excited to try new technologies that are both patient and provider-oriented.
- Excited to work with a collaborative team of physicians, APCs, registered dietitians, medical assistants, health coaches, and other staff.
