Part time, 20 hours per week, day shift. This position serves two 8-hour days at Mercy LIFE Assisi House (Aston, PA) and one 4-hour day at Mercy LIFE Valley View (Elwyn, PA). Days worked during the week are flexible. No night, weekend, on-call, or holiday coverage is required.

About the role

Under the supervision of the medical director, the Primary Care Physician (PCP) provides primary medical care to PACE Organization (PO) participants. Responsibilities include assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of the plan's effectiveness. The PCP functions in a collegial relationship with other healthcare professionals, making independent decisions regarding medical needs and health regimens, and carries out medical procedures as indicated.

Mercy LIFE Assisi House is a 125-bed, multi-level care facility, including an Alzheimer’s and Dementia unit, fostering a "person-centered care" approach. Mercy LIFE Valley View provides housing and the Mercy LIFE PACE program for senior adults, specifically serving a community of deaf seniors. Candidates must be willing to learn American Sign Language.

Responsibilities

  • Participate as a member of the interdisciplinary team (IDT) in initial, semi-annual, unscheduled, and annual assessments.
  • Report changes in participants’ baseline status to appropriate staff on a daily basis.
  • Provide initial and ongoing participant assessments, care plan development, and implementation of the care plan.
  • Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team.
  • Refer participants to medical specialists as indicated.
  • Manage all care of participants in the nursing home, including regular visits, collaboration with the IDT, and communication with nursing home staff for continuous monitoring.
  • Admit nursing home participants to the hospital when necessary.
  • Oversee and manage participants’ use of medical specialists and inpatient care; communicate and follow up with specialists regarding outcomes.
  • Review medical outcomes to establish best practices and improve participant health.
  • Complete documentation and use appropriate diagnostic coding.
  • Advise the Medical Director on improving accountability of PO services to participants and referral sources; keep the Medical Director informed of needed resources as the program expands.
  • Assist with the development of policies and procedures, standards of care, and ongoing monitoring and evaluation of patient care practices and service delivery.
  • Provide guidance and training to staff regarding medical and quality assurance issues.
  • Participate in provider on-call rotation as required.
  • Periodic travel within the Center's geographic region.

Requirements

  • Doctorate Degree (MD or DO).
  • Current and active plenary physician license to practice in the state of the PO.
  • CDS Certification and DEA Registration required.
  • Current BLS certification.
  • Board Certification in Internal Medicine or Family Practice; Board Certification in Geriatrics preferred.
  • Willingness to learn American Sign Language.
  • Minimum of one (1) year working with the frail or elderly population.
  • Valid driver’s license, proof of insurance, and use of personal vehicle required.
