Physician providing comprehensive care to seniors and individuals with complex medical and social needs within a Continuing Care Community.

Responsibilities

• Deliver primary and specialty medical care, managing chronic and acute conditions.
• Coordinate services across various settings including long-term care, rehab, home care, PACE, hospice, and community programs.
• Collaborate with an interdisciplinary team to ensure holistic, patient-centered care.
• Uphold dignity, cultural diversity, and Catholic values in all aspects of care to promote quality and safety.
