About the Role

We're seeking a compassionate, collaborative Primary Care Physician (MD/DO) to join our Old Fourth Ward practice in Atlanta, Georgia. This is an opportunity to practice evidence-based medicine in an environment that values collaboration, innovation, prevention, and meaningful patient relationships. You'll work alongside an interdisciplinary team including behavioral health providers, nutrition professionals, Advanced Practice Providers (APPs), care coordinators, and other clinical team members to deliver comprehensive, coordinated care.

Responsibilities

  • Provide comprehensive outpatient primary care services to patients across the lifespan
  • Diagnose, treat, and manage acute and chronic medical conditions using evidence-based clinical practice
  • Perform preventive health visits, annual wellness visits, routine physical examinations, health screenings, immunizations, and age-appropriate preventive care
  • Develop individualized treatment plans that integrate medical care with behavioral health, nutrition, preventive care, and lifestyle interventions
  • Identify opportunities to improve patient outcomes through Ascend's whole-person care model by leveraging behavioral health, nutrition services, care coordination, and other interdisciplinary resources
  • Collaborate closely with physicians, APPs, behavioral health providers, nutrition professionals, care coordinators, nursing staff, and other members of the care team to deliver coordinated, patient-centered care
  • Provide clinical oversight and collaborative supervision for APPs in accordance with Georgia law, organizational policies, and evidence-based practice
  • Support APPs through consultation, case review, mentorship, and clinical guidance to promote consistent, high-quality patient care
  • Coordinate care with specialists, hospitals, community providers, and other members of the patient's healthcare team to ensure seamless continuity of care
  • Engage patients through education, shared decision-making, and individualized care planning that empowers them to take an active role in their health
  • Maintain timely, accurate, and complete documentation within the electronic health record
  • Participate in quality improvement initiatives, clinical protocols, population health efforts, and best practices that support value-based care and excellent patient outcomes
  • Maintain compliance with all regulatory, clinical, quality, and organizational standards
  • Foster a welcoming, respectful, and inclusive environment for patients, families, and colleagues

Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
  • Board Certified in Internal Medicine or Family Medicine
  • Active, unrestricted Georgia medical license (or ability to obtain prior to start date)
  • Current DEA registration
  • Current BLS certification (or ability to obtain prior to start)
  • Strong commitment to evidence-based, patient-centered primary care
  • Excellent communication, collaboration, leadership, and interpersonal skills

Preferred Qualifications

  • Medicare enrolled (or eligible for enrollment)
  • Two or more years of outpatient primary care experience following residency
  • Experience working in team-based or value-based care environments
  • Passion for prevention, population health, and improving long-term patient outcomes
