About the role

A full-time Physician position focused on primary care within a multidisciplinary team, serving older adults in various community-based and outpatient settings. The role emphasizes team collaboration, geriatric and palliative care, and fostering patient independence.

Responsibilities

• Provide comprehensive, whole-person care, including medical, behavioral health, and recreational services.
• Conduct visits in private residences, assisted living, skilled nursing facilities, and clinics.
• Collaborate closely with a diverse interdisciplinary team of healthcare professionals.
• Build trust with patients and families, ensuring high-quality, patient-centered care.

Qualifications

• One year of post-graduate clinical experience with geriatric populations preferred.
• Training or experience in palliative care or hospice is valuable.
