About the role
Serves as a licensed practitioner responsible for comprehensive health care services in a health care setting.
Ensures patient and family-centered quality care to improve outcomes and patient experience, adhering to applicable policies, rules, and standards.
Responsibilities
- Manages patient care as outlined in practice protocols.
- Participates in the planning, coordination, and evaluation of patient care for acute Stroke interventions.
- Obtains thorough histories and performs physical exams to assess patients' physical and psychosocial health status.
- Provides routine health care maintenance services to patients.
- Affixes medical diagnoses and records and documents necessary data for coordinated health care.
- Initiates referrals/consultations and collaborates with attending physicians.
- Promotes safe quality patient care through intra/interdepartmental cooperation and collaboration.
- Communicates patient's plan of care with appropriate disciplines.
- Participates in Stroke Teams and evaluates Team Performance and outcomes.
- Administers NIH stroke scales and sees stroke patients anywhere in the organization.
- Contributes to the success of the stroke center by participating in call schedules that include nights and weekends.
- Fulfills professional practice responsibilities including adherence to guidelines for antiplatelet meds, DVT prophylaxis, anticoagulation, lipid measurement, dysphagia screening, use of NIH Stroke Scale, stroke education, and referrals to rehabilitation.
- Trains professional nurses and other staff in areas related to the clinical requirements and educational efforts of the Stroke Center.
- Makes follow-up calls to patients and arranges appropriate outpatient follow-up.
- Initiates and participates in studies and research.
- Participates in hospital committees.
- Self-develops by evaluating own practice, maintaining clinical competency, and participating in health-related research projects and performance improvement studies.
- Helps ensure compliance with mandated parameters of care.
- Participates in achieving TJC Certification as a Primary Stroke Center.
- Champions patient-centered care and the culture of safety.
- Demonstrates fiscal responsibility in the use of resources in the provision of patient care.
- Facilitates system changes to support the use of evidence.
- Participates in peer review process.
- Participates in the achievement of Hospital’s Strategic Goals.
Qualifications
- Minimum of 2 years experience as a licensed independent practitioner.
- Required current New York State registration as a licensed independent practitioner.
- Required certification by a national certifying body in her/his specialty area.
