PHYSICIAN ASSISTANT - POST ACUTE CARE

The Clinician in a Post-Acute Care setting provides comprehensive evaluation, treatment, and care management services to patients transitioning from acute hospitalization to rehabilitation, skilled nursing, long-term care, or home-based care environments. This role focuses on improving functional outcomes, managing chronic and complex medical conditions, reducing hospital readmissions, and supporting safe transitions of care. The clinician collaborates with interdisciplinary teams, including physicians, nurses, therapists, case managers, and caregivers, to develop and implement individualized care plans that promote recovery, independence, and quality of life.

Requirements

• Certification/degree from an accredited Physician Assistant program
• Two years of PA experience in a primary care practice, or a relevant specialty practice preferred
• Licensure/Certification(s): Current licensure as a PA-C by the State of Vermont. Certifications as an adult or family Physician Assistant by a national organization, which is recognized by the Vermont State Board of Medical Practice. Qualified to be credentialed by the BMH Medical Staff. Healthcare Provider CPR certification. DEA license
• OSHA Category: OSHA Category I - Tasks that involve exposure to blood, body fluids, or tissues require use of techniques of Universal Precautions as evidenced with blood borne pathogens standards
• Physical Demands: On-the-job time is spent in the following physical activities. Standing, Walking, Sitting, Talking or Hearing. This position requires that weight be lifted or force be exerted. Up to 30 pounds. This position has vision requirements. Close Vision (clear vision at 20 inches or less). Color Vision (ability to identify and distinguish colors). Ability to Adjust Focus (ability to adjust eye to bring an object into sharp focus). Hearing. Ability to hear calls. Ability to hear instructions from physician/department staff/others. Compensation

Brattleboro Memorial Hospital provides the compensation range for this position in good faith. The listed range represents the pay scale reasonably expected for a successful candidate based on factors such as relevant experience, education, internal equity, and market competitiveness. Final compensation will be determined during the hiring process. Salary Description $106,000-$122,000
