About the Role

We are a growing, physician-owned medical group expanding in the Duluth, MN area, seeking an experienced Physician Assistant (PA) to join our team in a Skilled Nursing Facility (SNF) setting. This role focuses on post-acute and rehabilitative care, including orthopedic recovery, stroke management, and pain management, working alongside a collaborative lead provider.

Key Responsibilities

  • Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning
  • Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions
  • Ensure accurate and timely documentation in EMR

Required Qualifications

  • Active Physician Assistant license in Minnesota and Wisconsin
  • Active DEA license (or ability to obtain prior to start)
  • Minimum 1 year of clinical experience as a PA (SNF, geriatrics, internal medicine, or rehab experience strongly preferred)
  • Experience managing adult/geriatric patient populations
  • Strong clinical decision-making and time management skills
  • Comfortable working independently in an SNF setting

Preferred Experience

  • Skilled Nursing Facility (SNF) or post-acute care experience
  • Background in internal medicine, geriatrics, or physical medicine & rehabilitation (PM&R)
  • Experience with orthopedic, stroke, or pain management patients

Physical Requirements

  • Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
  • Must be able to lift up to 50 pounds.
  • May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
  • Must wear the required PPE per company and/or facility requirements.
  • Must comply with facility vaccination requirements.
