The Health Center utilizes a multidisciplinary team approach with primary care providers, RNs, Social Workers, Psychiatry, Podiatry, etc. As a NP/PA at our Health Center, you'll have the opportunity to work alongside a multidisciplinary team of healthcare professionals to provide comprehensive care to our patients. Our Health Center utilizes a collaborative team model of care, integrating case management and behavioral health services to address the various medical and psychosocial factors impacting our patients' overall health. Job Duties

• Perform physical examinations, obtain medical histories, diagnose illnesses, develop treatment plans, and prescribe medications and therapies in accordance with standardized procedures and applicable scope of practice.
• Provide preventive and primary care services, including immunizations, health screenings, and wellness counseling.
• Manage chronic conditions such as diabetes, hypertension, asthma, and depression, with a strong focus on patient education and self-management support.
• Document all clinical encounters thoroughly and accurately in the Electronic Health Record (EHR) system.
• Collaborate with an interdisciplinary care team to address medical, behavioral, and social determinants of health.
• Provide acute care visits and triage support as needed.
• Serve as a preceptor for students or trainees when appropriate.

Qualifications

• Current license to practice as a Family Nurse Practitioner in California.
• Experience in outpatient primary care, preferably in a community health center setting.
• Strong interpersonal and communication skills.
• Ability to work collaboratively with a multidisciplinary team.
• Knowledge of clinical protocols, policies, and regulatory requirements.
