About the Role
We are seeking a Full Time (40 hours per week) provider for our Autumn Lake Healthcare at Glade Valley Center located in Walkersville, MD. This position can be filled by a nurse practitioner or physician assistant. AlignMed Partners is a national medical group dedicated to improving health outcomes among residents of skilled nursing and senior living facilities.
Responsibilities
- Reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
- Collaborates on-site with the Center Leadership Team for center-specific goals and priorities.
- Collaborates with the Medical Director and/or Attending Physician for patient-specific medical care.
- Provides primary and urgent care services to nursing center residents in collaboration with other medical providers in the center.
- Works collaboratively with Center Leadership to achieve AlignMed Partners' mission of improving the lives we touch through the delivery of high-quality healthcare and everyday compassion.
Qualifications
- Nurse Practitioner: Graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing. Concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP) required. Other concentrations/specialty tracks considered based on experience and business need.
- Physician Assistant: Graduate from a program accredited by the ARC-PA and must pass the PA National Certifying Exam (PANCE) to use the PAC designation. PA state licensure and NCCPA certification required.
- Minimum of one year of Advanced Practice Provider (APP) experience required; two years of experience working with adult or geriatric population preferred.
- Unrestricted professional license and DEA for each state in which they practice (where state practice allows).

Restrictions apply based on collective bargaining agreements, applicable state law, and factors such as pay classification, job grade, location, and length of service. Eligibility for full-time benefits requires a minimum of 30 hours per week. Certain benefits, including Health plans, are not available to part-time employees working between 16 and 29 hours per week.
Pay
Base salary range: $114,000 - $125,000 per year.
Schedule
Full-time, 40 hours per week. No on-call or weekend shifts.
