Requirements
- Licenses as a Medical Doctor or Doctor of Osteopathic Medicine in Washington State
- Board eligibility or board certification by the American Board of Anesthesiology or the American Osteopathic Board of Anesthesia
- Prescription authority licensure in State and the DEA
