Physical Therapy Assistant, PTA - Home Health - PRN

About the role

As a Physical Therapy Assistant (PTA) at VitalCaring, you will provide one‑on‑one, home‑based care to help patients regain strength, mobility, and independence. You will work under the direction of a licensed Physical Therapist, delivering personalized therapy that directly impacts recovery and quality of life.

Responsibilities

• Deliver high‑quality patient care by providing direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, functional mobility, transfers, strengthening, and activities of daily living.
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
• Observe and report patient progress, response to treatment, and any changes in condition.
• Maintain accurate, timely documentation in the EMR.
• Communicate effectively with the Physical Therapist and interdisciplinary care team; participate in case conferences and care coordination efforts.
• Collaborate with nurses, therapists, and other healthcare professionals to ensure coordinated care.
• Participate in team meetings, in‑services, and ongoing training; uphold clinical standards and contribute to quality improvement efforts.

Requirements

• Graduate of an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as required by state law.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• 1+ year of experience preferred; home‑health experience a plus.
