About the role

Responsibilities

  • Conduct in-home visits for clients suffering from chronic and terminal illnesses, following the physician’s Plan of Care.
  • Document all services provided, observations, client progress, and recommendations on designated forms and submit within seven days.
  • Report changes or schedule changes to the therapist and/or nursing supervisor promptly.
  • Participate in interdisciplinary activities and attend client care conferences as needed.
  • Promote referrals to other disciplines as appropriate.
  • Attend staff meetings and continuing education programs.
  • Participate in Agency quality improvement activities.
  • Maintain confidentiality and adhere to safety and infection control practices.
  • Perform additional duties as deemed necessary.

Requirements

  • Current license, registration, or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the physical therapy examination.
  • Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
  • Minimum of six (6) months experience as a Physical Therapy Assistant.
  • Demonstrated written, verbal communication skills, and interpersonal skills.
  • Licensed driver with a clean driving record and insurance in accordance with state and/or Agency requirements.

Qualifications
