About the role

Award-winning in-home care provider, Professional Case Management, is seeking dedicated Physical Therapy Assistants to support our clients in New Mexico. Our team provides comprehensive, personalized care to individuals affected by past work environments, offering flexible schedules and competitive compensation.

Responsibilities

  • Conduct in-home visits for clients, following the physician’s Plan of Care and therapy care plan.
  • Document all services provided, observations, client progress, and recommendations on designated forms and submit within seven days.
  • Report changes or schedule changes to the therapist and/or nursing supervisor promptly.
  • Participate in interdisciplinary activities and client care conferences.
  • Promote referrals to other disciplines as needed.
  • Attend staff meetings and continuing education programs.
  • Engage in Agency quality improvement activities.
  • Maintain confidentiality and adhere to safety and infection control practices.
  • Perform additional duties as required.

Requirements

  • Current license, registration, or certification as a Physical Therapy Assistant in the state(s) of employment or eligibility to take the physical therapy examination.
  • Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
  • Minimum of six (6) months experience as a Physical Therapy Assistant.
  • Demonstrated written, verbal communication skills, and interpersonal skills.
  • Licensed driver with a clean driving record and insurance compliant with state and/or Agency requirements.

Qualifications

  • Current license, registration, or certification as a Physical Therapy Assistant in the state(s) of employment or eligibility to take the physical therapy examination.
