About the role

The role involves providing physical therapy assistance to patients in a home care setting, following the established plan of care set by a supervising Physical Therapist.

Responsibilities

• Instructing patients on exercises, muscle re-education, gait, ADL, transfer, safety, and prosthetic training
• Observing and reporting patient responses and condition changes
• Coordinating care with healthcare team members
• Educating patients, families, and caregivers within the scope of practice

Qualifications

• Current licensure or certification to practice physical therapy
• Valid driver's license and transportation
