Responsibilities
- Conduct in-home visits for clients following the physician's Plan of Care.
- Document all services provided, observations, client progress, and recommendations.
- Report changes or schedule changes to the therapist and/or the nursing supervisor.
- Participate in interdisciplinary activities and client care conferences.
- Promote referrals to other disciplines as appropriate.
- Attend staff meetings and continuing education programs.
- Participate in Agency quality improvement activities.
- Maintain confidentiality and adhere to safety and infection control practices.
- Perform additional duties as deemed necessary.
Requirements
- Current license, registration, or certification to practice as a Physical Therapy Assistant in the state(s) of employment.
- Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
- Minimum of six (6) months experience as a Physical Therapy Assistant.
- Demonstrated written, verbal communication skills, and interpersonal skills.
- Licensed driver with a clean driving record and insurance in accordance with state and/or Agency requirements.
Qualifications
