Primary Responsibilities

• Follow state-specific laws governing physical therapy in home care
• Perform treatment and provide exercise instruction and patient education
• Instruct and aid patients in various forms of exercise and training
• Observe, record, and report patient responses and changes in condition to supervising Physical Therapist
• Coordinate care with other members of the healthcare team
• Instruct family, caregivers, and other healthcare team members in therapy areas

Required Qualifications

• Current, licensed, registered, or certified physical therapy assistant license in state of practice
• Current driver’s license and vehicle insurance, or access to public transportation

Preferred Qualifications

• CPR certification or ability to obtain within 90 days of hire
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility

Pay Range

$58,241 - $87,361 annual total cash target pay
$28.00 - $42.00 per visit point
$33.60 - $50.40 hourly rate
