Physical Therapist (Unit) - $7,500 Sign on Bonus

About the role

We are seeking a compassionate Physical Therapist (PT) to join our pediatric therapy team in Fountain, Colorado. The ideal candidate will provide care in home health settings and will be part of a dedicated team that matches therapists to patients based on medical condition, age, location, and acuity level.

Responsibilities

• Provide physical therapy services to medically fragile children and adults in their homes
• Develop and implement individualized treatment plans based on patient needs
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Document progress and communicate effectively with patients, families, and healthcare providers

Requirements

• Must be licensed to practice Physical Therapy in the current state of practice
• Must be willing to work in home and clinic-based settings (state specific)
• Valid, unexpired Driver’s License and automotive insurance
• Experienced in pediatric rehabilitative services preferred

Qualifications

• Masters degree in Physical Therapy from an accredited program
• Licensure as a Physical Therapist in the state of practice
• Current CPR certification

• Knowledge of pediatric rehabilitation principles

Pay

• $75.00-$85.00 per visit
