Physical Therapist - Travel Contract

Join a traveling Physical Therapist role providing home health services in Lewistown, PA, focusing on patient recovery and chronic condition management in community settings across Mifflin and Juniata Counties.

Responsibilities

• Deliver direct physical therapy care in home health settings
• Provide patient education and support for recovery and chronic condition management
• Coordinate with interdisciplinary teams
• Use electronic documentation systems such as HCHB and OASIS

Requirements

• Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program
• Active Pennsylvania Physical Therapy license
• BLS certification (may be required)
• Preferred experience in home health and familiarity with home visit workflows
• Valid driver’s license and ability to travel within assigned areas

Schedule

• Monday through Friday, daytime hours
• Guaranteed 32 hours weekly
• No on-call or weekend duties required
• Travel between patient homes with mileage reimbursement
• Dress code: blue scrubs
