Physical Therapist - Travel Contract

Role: Traveling Physical Therapist specializing in home health services. Responsibilities: Conduct SOC, ROC evaluations, recertifications, and revisits; document visits using HCHB; support patient recovery and independence; ensure regulatory compliance. Qualifications: Bachelor's, Master's, or Doctoral degree in Physical Therapy; active Indiana license; BLS certification (may be required); home health and HCHB experience; experience with SOC, ROC, recerts, and revisits. Preferred Skills: Travel therapy experience; familiarity with St. Joseph and Marshall County markets. Key Duties: Provide high-quality home health care within designated territories; work independently in a home environment; complete documentation accurately.
