Role & Responsibilities
- Provide patient evaluations and skilled therapy services in patients' homes across designated counties.
- Collaborate with interdisciplinary teams to support discharge planning and continuity of care.
- Document patient outcomes to enhance quality measures and satisfaction.
- Travel within coverage areas, offering comprehensive home-based therapy interventions to improve mobility, function, and safety.
Qualifications & Skills
- Degree in Physical Therapy (Bachelor’s, Master’s, or Doctoral) from an accredited program.
- Active Kentucky physical therapy license.
- Basic Life Support (BLS/CPR) certification.
- Ability to perform evaluations and work independently in home health settings.
- Willingness to travel within designated areas.
Additional Details
- No on-call or weekend shifts required.
- Mileage reimbursed between patient visits.
