Physical Therapist - Travel Contract

About the Role

Seeking a traveling Physical Therapist to deliver home health services in a rural area, supporting patients recovering from injuries and managing chronic conditions. The role involves providing skilled evaluations, developing treatment plans, monitoring progress, and collaborating with interdisciplinary teams to ensure safe, functional outcomes.

Responsibilities

• Provide in-home physical therapy care across multiple small towns and lake communities.
• Coordinate patient care in their homes and communicate with physicians and caregivers.
• Manage a caseload in a rural setting, traveling between patient locations as needed.
• Ensure safe and effective patient recovery and independence.

Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Wisconsin physical therapy license.
• At least 12 months of recent home health physical therapy experience.
• BLS certification (may be required).
• Experience with electronic medical records preferred.

Preferred Attributes

• Prior travel or agency home health experience, especially managing rural caseloads and traveling between patient homes.

Schedule

Standard hours are 8am-5pm, Monday through Friday, with no on-call duties. Travel within the territory is expected.
